He was 49 years old and came to the hospital emergency department at 1 a.m. with abdominal pain for 4 hours. In the medical history he provided, he described that when he and his wife were having sex, he suddenly felt severe abdominal pain, which then became increasingly severe, followed by symptoms such as nausea, vomiting, and fever. Physical examination: Respiration: 25 times/min, body temperature: 38.5℃, heart rate: 85 times/min. The abdomen is board-like, the entire abdominal muscles are tense, and there is obvious tenderness and rebound pain. B-ultrasound showed a small amount of fluid in the abdominal cavity. Abdominal puncture obtained a small amount of bloody and purulent fluid. There were no abnormal findings on abdominal plain radiographs. The patient insisted that his illness was related to sex, but I couldn't figure it out. Could sex cause abdominal pain? At this time, the patient had obvious peritonitis and the puncture was positive, so there was an indication for exploration. It was decided to perform laparoscopic abdominal exploration immediately. Under laparoscopy, intestinal contents and bloody pus were found in the abdominal cavity. There is a hole in the jejunum, which is obviously an intestinal rupture caused by trauma. What causes the intestinal rupture? Continuing to search for the cause in the abdominal cavity, we found that the right internal ring opening was significantly enlarged. (As shown in the picture!) It turns out that he originally had a right inguinal oblique hernia! (This is the internal ring opening of the right inguinal oblique hernia. The intestine protrudes from this opening into the subcutaneous tissue.) When the couple was making love, he was on the bottom and she was on top. As soon as he exerted force, an oblique inguinal hernia appeared on his right side, which meant that the intestine protruded into the groin. (As shown below) His wife was rather fat and moved violently, and when she sat down on him she broke his intestines that were protruding under his skin. This is why having sex can also rupture a man's intestines. Since the first time I encountered this type of patient, I have encountered 4 similar patients. The intestinal rupture was repaired under laparoscopy and the internal ring opening was ligated at a high position. The operation was successful and the patient recovered quickly. Patients with hernia, regardless of gender, should be careful not to accidentally injure themselves when having sex. Of course, patients with hernia are also advised to go to the hospital for surgery as soon as possible to avoid accidents. |
