In clinical diagnosis, one should distinguish between the deficiency and excess of the disease. The diseased internal organs are a mixture of deficiency and excess. For excess symptoms, the treatment should be tonic; for deficiency symptoms, the treatment should be purgative. Temporary erectile dysfunction caused by fever, excessive fatigue, abnormal emotions, etc. cannot be regarded as a pathological condition. The symptoms of impotence were first recorded in the Yellow Emperor's Classic of Internal Medicine. The chapter "Evil Qi and Organ Disease Forms" in Lingshu called impotence "penis impotence". It was also called "chastity of the tendons" and "tendon impotence" in Suwen. It is believed that asthenia and pathogenic heat are the main causes of impotence. "Lingshu·Jingjin" points out: "Heat causes the tendons to relax and not contract, and the penis becomes impotent and cannot be used." Doctors in the Sui, Tang and Song dynasties believed that impotence was caused by overwork and kidney deficiency. For example, "Zhubingyuanhoulun·Xulaoyangwuhou" believes: "Overwork injures the kidneys, and kidney deficiency cannot nourish the genitals, so they become weak. ” Therefore, the treatment focuses on warming the kidney and strengthening yang. Causes 1. Lack of talent, long-term illness due to overwork Congenital deficiency or unbridled indulgence in sexual desire, excessive sexual intercourse, masturbation, or early marriage can all cause loss of vital energy and weakening of the fire in the life gate, leading to impotence. In addition, long-term illness and overwork can damage the spleen and stomach, and insufficient qi and blood can cause the tendons to be malnourished and lead to impotence. 2. Disorders of the Seven Emotions If emotions are not fulfilled, if thoughts and desires are excessive, if one is worried, depressed and angry, the liver will not be able to vent. The gathering of the main tendons makes them powerless, resulting in impotence. Too much worry may damage the heart and spleen, resulting in insufficient qi and blood, and malnutrition of the tendons; or sudden fright may damage the heart and kidneys, causing qi disorder, and qi and blood failing to reach the tendons, making them unable to strengthen and causing erectile dysfunction. 3. Eating disorderly Excessive consumption of strong alcohol and rich foods will cause the spleen and stomach to function abnormally, causing dampness to accumulate and generate heat. The dampness and heat will flow down to the liver and kidneys, blocking the meridians and preventing qi and blood from nourishing the tendons, thus causing impotence. 4. External invasion Living in a wetland for a long time or invading the liver with dampness and heat, which accumulate in the liver meridian and flow down to the tendons, or cold and dampness which damage the yang, causing the yang to be suppressed by the yin and resulting in impotence. Pathogenesis The basic pathogenesis is damage to the liver, kidneys, heart, and spleen, resulting in deficiency of qi, blood, yin and yang, and loss of nourishment to the yin meridians; or liver depression and dampness obstruction, which lead to obstruction of the meridians and cause the dysfunction of the tendons. The disease is located in the tendons, and the affected organs are mainly the liver, kidneys, heart, and spleen. Pathological nature: There are distinctions between deficiency and excess, and often both deficiency and excess occur simultaneously. Key points of examination 1. When an adult male has sexual intercourse, his penis becomes flaccid and cannot be erected, or it is erected but not firm, or it is firm but not for long, making it impossible to have a normal sexual life. However, sexual dysfunction caused by penile hypoplasia must be excluded. 2. Common symptoms include fatigue, sore waist and weak knees, fear of cold limbs, restless sleep, depression, timidity and suspicion, or difficulty urinating and dripping. 3.
